Dr.Salem is a Certified Functional Medicine Practitioner and her practice focuses on chronic health issues. She works with the patients to help them determine the possible root cause(s) of their health problems. This does not include annual health checkups and acute illnesses like colds and sore throat.

 

Here is an overview of the general process:

 

1. Complete a detailed questionnaire about the patient's health history and needs

2. Dr. Salem will review the questionnaire before the consultation

3. Schedule a consultation (tele-health)

4. During consultation, discuss the health history and about additional labs as needed

5. Complete the Lab tests

6. Follow up once the lab tests are ready, to discuss treatment options.

7. Ongoing follow ups as needed, depending on the health issue being addressed.

The Initial Consultation involves reviewing the Detailed Intake form with the patient/parent, getting additional history so we can chart a Timeline of events. This can help us get to the root cause(s) of the current health condition.  We will also discuss any Lab tests that may help us gather more information. Total time allocated is about 120 min with 60 min of health information review prior to the visit and up to 60 min of actual consultation time.

 

A follow up visit is scheduled once the recommended Lab reports are ready. During this Lab follow up visit, treatment options, including diet and lifestyle as well as supplements will be discussed.

 

Additional follow ups after this will depend on the patient’s progress and the health condition that is being addressed.

 

Price and Payment Option: We do not accept any Commercial Health Insurance, Medicare or Medicaid at this time. Our regular price for this consultation service is $299. For a limited time, we are offering a special discounted price of $250. Pre-payment is required prior to consultation. This does not include the cost of the Lab tests or supplements.

 

You will also have access to our online portal wherein you will be able to manage all your information including forms, labs, medical records and communication. You will also have the ability to download our app and manage the same via a mobile device.
