We’re so glad you’re interested in working with Dr. Salem. She is a Certified Functional Medicine Practitioner, and her practice focuses on helping patients identify and address the root causes of chronic health concerns.

Please note that her services do not include annual checkups or short-term illnesses such as colds, flu, or sore throats.

Healing from chronic conditions is a process that takes time and guidance. To best support you throughout this journey, we’ve created a comprehensive care package that includes:

Health Questionnaire – You’ll complete a detailed form about your health history and current concerns.
Consultation Review – Dr. Salem personally reviews your information before your first visit.
Initial Consultation (30–60 minutes, via Telehealth) – Together, you’ll review your health history and outline a personalized care timeline.
Lab Testing (as recommended) – Any necessary labs that will help uncover underlying issues.
Lab Review & Treatment Planning – Once lab results are available, Dr. Salem will meet with you via Telehealth to discuss a tailored plan that may include nutrition, exercise, supplements, and lifestyle recommendations.
Ongoing Support – You’ll receive continued follow-ups and secure messaging through our HIPAA-compliant patient portal, depending on your individual needs.

Follow-Up Visits

Additional follow-ups are based on your progress and health goals.

Pricing & Payment

We currently do not accept Commercial Health Insurance, Medicare, or Medicaid.

Our 3-month Functional Medicine Healing Journey is $499, which includes consultations and ongoing messaging support.

If you need continued care beyond that, follow-up packages are available for $350 per 3 months.

Payment is required prior to your consultation.

Please note that lab tests and supplements are billed separately.

Patient Portal Access

You’ll have full access to our online patient portal, where you can securely manage forms, lab results, medical records, and communication. You can also use our mobile app for convenient access anytime.
